The Connected Care Community, made up of physician groups, ACOs, and hospice organizations, is a new mechanism that provides both hospice organizations and physician groups with the tools and data they need to introduce hospice care, with all its palliative care benefits, to patients sooner in their end-of-life journey. Through Connected Care Communities, hospice organizations assist physician practices in managing patients with advanced illnesses by obtaining referrals further upstream, allowing physician practices to manage patient costs more effectively, reduce their uncompensated work burden, and match their patients’ goals for their final months, weeks, or days.
